Overview

The  integration  of  the  Health  and Social Care  sub-sectors  is  important in  terms  of being able  to  provide  services  for the  wellbeing of  individuals,  and to  meet  the increasing  demands  of  a  growing and ageing population  with  increasingly  complex needs.  Students  working  in  health  will need to  be  aware  of  integrated care  pathways: a  multidisciplinary  approach  towards  anticipated care  that enables  an  individual with identified  needs  to  move  progressively  through  their journey  and  experience positive outcomes. The  aim  of  this  unit  is  to  develop students’  understanding of  an  individual’s  right to being  involved in  their own  care  and develop students’  skills  in  promoting  this  right when  working  with  individuals.  This  right is,  in  many  cases,  enshrined  in  law  and  in  the fundamental  standards  of  care. It  is  a critical  element  of  person-centred  care and leads  to  improved and often  more  cost-effective outcomes.

Modules

  1. Examine the health, care and support services available to an individual requiring multidisciplinary care
  2. Assess an individual’s capacity to identify their own needs
  3. Describe the impact of own relationship with the individual and multidisciplinary teams involved in the delivery of the care pathway
  4. Demonstrate the need for person-centred communication in implementing person-centre plans

Career Progression

This unit counts towards RPL for a level 4  Healthcare Practice - Level 4 HNC (Pearson)  (or higher Apprenticeship)

Successful students can progress to a wide range of degree programmes including Nursing, Teaching, Public Services and Health Care, Social Work, Youth and Community Work and Psychology and Counselling.

Aspire Eligibility

All programmes are open to Londoners who meet the following criteria:

  • UK resident and living in London
  • Aged 18+
  • Do not hold a level 4 (post-college/sixth form) qualification
  • Either employed (and earning less than London Living Wage) or unemployed/economically inactive

What is expected of me?

- To be an active participant in the programme.

- To complete all of the workbooks funding paperwork and the accredited unit.

- To undertake a workbased project (will be supplied by us if you are currently not working)

- To participate in all the group online workshops and 1-2-1 coaching sessions.

How much does this cost?

Participation in this programme is completely free. It is fully funded by the European Social Fund (ESF) and the Greater London Authority (GLA).

Additional support for travel to and from places of study and other expenses will vary depending on the provider and the support they are able to offer. These options will be discussed during your enrolment.

How long will it take?

The part time programme typically takes 8 - 12weeks to complete

Can I take more than one unit?

You cannot study more than one unit on the Aspire London Programme.  However, upon completion of your programme, you’ll get advice and guidance from your Aspire Careers Coach about how and where you can continue to study, if this is the next step for you and your career progression. This could include a full Level 5 qualification.
